Thread
I am not a key opinion leader, and I do not take any money from pharma.

It is likely my opinion is irrelevant, but I thought I would offer the unfiltered truth about major pharma companies that make #mmsm drugs in a humorous and authentic way. Free consulting in this 🧵
Janssen:

Listen bro, daratumumab is a great drug. You dont need me to say it.

But your greed in giving daratumumab indefinitely, and where it hasnt been shown to really help (such as continuous maintenance after dara containing induction) nauseates me.
Sanofi/Isatuximab:

I wish you had a niche you could market. But the reality is that other than a "slightly" cheaper price if given for a short duration, isatuximab offers zero advantages over daratumumab. Your Gain1q angle is marketing masquerading as precision medicine. Sorry.
Oncopeptides/Melflufen:

Listen, I appreciate that as a small biotech company, you made it this far. But it is sad- that your drug had to KILL more people in one trial (OCEAN), in order to STOP a grossly unethical second trial (the LIGHTHOUSE trial)! IRBs and KOLs let it happen💔
BMS/Celgene/Lenalidomide:

Your drug has worked so well for so long! Your refusal to let your drug have easy cheap generics in America is a posterchild for corruption. Also, I see that as len becomes generic, you are replacing with iberdomide (without a direct comparison) 👀
Belantamab/GSK:

Listen, I honestly believe your drug is better than selinexor and I prescribe it. But a drug that affects vision and severely reduces quality of life doesn't belong in maintenance. Or smoldering myeloma. Please dont do such trials. Id never enroll my mom on them.
Karyopharm/Selinexor:

As someone said so eloquently- youre a drug looking for a disease.

I know that a KOL once wrote in a paper that selinexor is the next checkpoint inhibitor because it works in so many diseases, but thats the worst insult to immunotherapy I have EVER heard.
Amgen/Carfilzomib.

Listen man- you tried real hard. Multiple randomized trials to try to beat Velcade in the front-line setting. But you failed. Time to accept it and move on. Also, in the real world, your drug does make people very short of breath. I hope you know that.
I think thats enough for this thread. Free consulting for all the world to see (not that my opinion matters).
I mean no offense, and I hope to generate good conversation and thought. Obviously some nuance is lost. Thanks!
